All new enrollments must include:
- Birth Certificate
- Immunization Record
- Proof of Residency (Driver's Licence, bill or official document with address)
Print and complete:
Elementary
Middle School
High School
Little Hawks Academy
Student Accident Insurance for 2023-24 SY

On the "Enroll Now" option, choose "K12 Voluntary" then type in MSD of Boone Township for the District Name and choose Indiana.
